The risk isn't the liposuction or the fat transfer itself — it's where the fat is injected. If fat is injected into the gluteal muscle, it can enter a large vein and travel to the lungs, causing a fat embolism. This is the complication that led major surgical societies worldwide to publish updated safety guidance a few years ago. The fix is well-established: fat should only be placed above the muscle, in the subcutaneous layer, ideally guided by ultrasound so the surgeon can see exactly where the cannula tip is at all times.
What a safety-conscious clinic actually does differently

The recovery timeline, realistically
